Presenting Complaint :
"Soul-ache" - unease sensation on the slight left side of the chest with severity rated 10/10 with 10 being the worst. 'Pain' radiated to whole body resulting in feeling of low. 'Pain' was brought on by the thought of being away and abandoned by God. Patient realised how little of good deed and ma'ruf effort, if any, that had been done and the abundantly-ever-growing sins committed. The fact that patient was actually had done nothing to improve the situation aggravated the 'pain'.

History of presenting complaint :
Patient has experienced mild deterioration in life in general for the past few couples of week.

Past Surgical History : NAD

Medication :
"Repentenzin" 175mg
"Do'ax" 150mg
"Tahajjudol" 300mg
Poor compliance to all three

Allergies : NKDA

Family History :
All family members are well

Social History : NAD

Student's notes :
Patient was advised to take the medication as instructed. "Tahajjudol" has proven to be very effective in eradicating signs and symptoms of "soul-ache" and further preserve the calmness and serenity within patient. This triple therapy [Tahajjudol, Repentenzin and Do'ax], amazingly, has no side effects and does not inflict contraindication in all type of patient. Ancient findings nor recent scientific studies are able to challenge the astonishing capability of this therapy, which its effectiveness increases exponentially with the dose taken.
